Role description

As an Advanced Clinical Pharmacist you will take the lead, in your clinical area of expertise, for the delivery, co-ordination, expertise and clinical skills needed to provide care to people who would previously have required hospital-based treatment.

You will work closely with colleagues from primary, secondary and third-party care organisations, to improve the outcome for patients and ensure the safe and optimised use of medicines across the area.

Key responsibilities

  • Provision of prescribing advice and support to medical and nursing staff in outpatient clinics and at ward level
  • Supporting and educating patients about the safe and optimal us of their medicines over the phone and in an outpatient setting.
  • Taking clinical decisions about adverse drug reactions or abnormal blood results
  • Establishing and developing a clinical pharmacy service (DMARD prescribing and monitoring service) within clinical areas and monitoring outcomes
  • Develop medicines related policies and protocols
  • Liaising with the clinical commissioning regarding the use of high-cost medications

Qualifications

  • A clinical pharmacy diploma, or demonstrable equivalent clinical pharmacy knowledge.
  • Excellent communication and inter-personal skills
  • Good IT skills are necessary and a working knowledge of GP clinical systems would be an advantage.
  • Change management, project management experience
